What you will need
- Full legal name — as it appears on your government-issued ID.
- Date of birth — month, day, year.
- Mailing address — current address on file with the FAA.
- SSN or certificate number — your FAA certificate number is preferred. SSN is optional but may speed processing.
- Airman certificate type & number — if you hold one.
- Occupation — your current job title.
- Employer — if applicable.
Tip
Fill out this section on the actual MedXPress website (medxpress.faa.gov) shortly before your AME appointment — not weeks in advance. Your application is only valid for 60 days once submitted. Have your information ready to enter but wait to submit.
