Section 1: Personal Information
FAA Form 8500-8 Items 1 – 16
What you will need
- Full legal name — as it appears on your government-issued ID.
- Date of birth — month, day, year.
- Mailing address — current address on file with the FAA.
- Social Security Number (optional) — the SSN is not mandatory. If you don't want to provide it, check “International/Declined to Submit” (Item 4).
- Airman certificate type & number — if you hold one.
- Occupation — your current job title.
- Employer — if applicable.
- Item 13 — whether any FAA medical certificate has ever been denied, suspended, or revoked. If yes, give the date and details. An AME can't issue a certificate after a yes unless you bring FAA evidence of later certification or the AME gets FAA authorization.
- Item 16 — date of your last FAA medical application, even if it was many years ago or didn't result in a certificate. An estimate is fine.
Tip
Fill out this section on the actual MedXPress website (medxpress.faa.gov) once you know when your AME appointment will be. After you submit, your exam must take place within 60 days or the application is deleted, and an application you don't submit within 30 days is removed. When you submit, you'll get a confirmation number. Bring it and a government-issued photo ID to your exam.
